CDC National Hypothesis Generating Questionnaire [Updated 2023]

The Standardized National Hypothesis Generating Questionnaire (NHGQ) is a tool designed for use during the hypothesis generation phase of a multistate foodborne or enteric zoonotic outbreak investigation. The questionnaire provides a standardized method for the rapid collection of exposure information across multiple state and local jurisdictions. The goal of the NHGQ is to identify one or more leading hypothesis which then can be further investigated in the hypothesis testing phase of the outbreak investigation.
